Please complete all areas to the best of your knowledge. The more information you can provide the faster we can locate your original certification records. Some of this information can be found on your ACUC Diploma, in your Log Book, or by contacting a person you took your course with. Please complete and send either form to ACUC BEFORE you telephone our office. Our staff can only assist you after they receive this form. |
