If you were certified in Canada, USA, or in any English or French speaking country in the Caribbean please use this Replacement Card Application Form. If you were certified anywhere else in the world, including Cuba, please use this form. To replace ACUC Divemaster or Instructor card please contact your ACUC National Office for details.

Please complete all areas of the form to the best of your knowledge. If you do not know your ACUC registration number please do not worry, our staff can process your application without it. Confirmation of your membership is generally available 2 business days after we receive your completed application for Open Water Diver, Advanced Diver, or Rescue Diver levels. Specialty Diver levels registered with ACUC prior to 1995 may take up to 10 business days to verify. We appreciate your patience. A picture of you is required for your new certification card. When choosing your picture please be sure that you are the only person in the photograph and that it shows your face clearly. Pictures may be submitted by email in JPG format or by mail. Because of poor resolution we cannot accept pictures submitted by fax. We accept Visa, MasterCard or personal check/money order for payment. If you are not comfortable providing credit card information by email you may telephone our office. Your new certification card is shipped to you via Canada Post regular mail service after your payment has been processed. Please allow 10 business days for processing and shipping after you receive your Membership Confirmation Letter by email.
