New Diver Registration Form

ACUC is excited about our new Diver Registration Form.  Now it is even easier to complete and submit to our office, and you don’t even need to be online* to use it.

Our new Diver Registration form is in Adobe PDF format.  You can save it to your computer, complete it at your leisure, and when you’re ready, submit it to our office using your email application.  You can even mail it to us instead of you prefer.

· Only current, active, ACUC Instructors may use this form.  Forms submitted by other individuals who have not been preauthorized by ACUC will be discarded.

· Fees for new certification cards are published in the current ACUC Instructor Price List.  Please contact info@acuc.ca if you require another copy.
